Employment Type:Full timeShift:Description:Position Purpose:Physical Therapist, PTs provide professional physical therapy services to the organization’s clients as prescribed by the physician, and in compliance with the state’s professional practice act for physical therapy, any applicable licensure/certification requirements, and the organization’s policies and procedures.Position Highlights:

Accurately assesses clients at admission and other required time points, and utilizes appropriate standardized documentation. Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.

If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.

Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment.

Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients.

Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.

Minimum Qualifications:

Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.

Licensed to practice as a physical therapist in the state of practice.

Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum of 6-8 weeks.

Current working knowledge of conditions of participation and home healthcare experience preferred.

Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with agency’s clients and families.

Must have current Driver’s license and reliable transportation to and from work site.
