-Analyze clinical data and identifies Hierarchical Condition Category (HCC) gaps.
-Query/communicate with provider/care manager to capture accurate Hierarchical Condition Category (HCC).
-Assist in the evaluation of data collected to assure accuracy and reliability. Prepares reports and performance reporting of quality data.
-Oversees quality reporting to the insurance carriers.Minimum Qualifications:Minimum Education
