The position at UPHS Bell monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. This position investigates, assesses and plans interventions to help patient through rehab process.Acting as a patient representative will interact with the medical staff, hospital staff, vendors of other health care services to ensure efficient, cost effective health care that is compliant with regulatory agencies and LifePoint standards. Collaborates with multidisciplinary team regularly to evaluate patient’s status and appropriateness of medical care, including admission, length of stay, transfer and discharge.Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members and payors regarding care.Participates in discharge planning including coordination of patient placement to other facilities and community resources. Provides discharge education and resource referrals to patients.Ensures authorizations for hospital services are obtained and documented. Assists with verification of medical necessity requirements.Follows infection control and safety guidelines.Assists in coordination of swing bed utilization.Interviews and assesses patients and/or patients family, caregivers and/or legal representatives.Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor and evaluate compliance with patients treatment plan.Assists with discharge planning and processes. Identifies appropriate resources including transportation, housing, healthcare and social/spiritual services, and provides referrals as part of the discharge plan.Marketing and off site visits to promote and continue swing bed programAble to function in other areas of case manage/discharge planning and care plan management of the acute care patient.Case Manager is responsible for facilitating appropriate health care services throughout the continuum of care. Uses collaborative and systematic processes to assess, plan and coordinate the implementation of services that meet the individual's health needs. Collects and utilizes data to monitor and evaluate the appropriateness and effectiveness of those services in accordance with the organizations PI model. Communicates with physician, payers and others health care providers to ensure quality care and promote fiscal responsibility through the appropriate utilization of resources. In the absence of a dedicated Clinical Documentation Specialist the Case Manager utilizes advanced knowledge and clinical expertise to direct the health care providers towards the improvement of clinical documentation. Facilitates the overall quality, completeness and accuracy of the medical record. Through extensive interaction with physicians, nurses, and other care givers, achieves appropriate documentation to support the medical necessity and level of services rendered to all patients. Assist with the ongoing development and delivery of education on documentation guidelines, coding and reimbursement rules for all members of the patient care team.Participates and attends meetings as required and/or assigned

Performs other related duties as assigned or requestedMinimum Education

Graduate of a nursing program.

Preferred: Bachelors of Science in NursingRequired Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Required Skills

Licenses: Current license to practice as an RN in the State of Michigan.
