Social Services AssistantAuburn Homes & Services, Chaska, MNCompensation: $17.25- $19.00Employment Type: PT Schedule: 3day a weekThe primary purpose of your job position is to assist in developing, implementing, evaluating, and directing our facility’s social services in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and psychosocial needs of the resident are met and maintained on an individual basis.Works in office areas as well as throughout the facility.Moves intermittently during working hours.Is subject to frequent interruptions.Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.Is subject to hostile and emotionally upset residents, family members, etc.Communicates with the medical staff, nursing service, and other department directors.Maintain a social service directory listing available community resources.Assist with discharge planning and make referrals to community agencies as appropriate. Work with the facility’s consultants as necessary and implement recommended changes as required.Make routine visits to residents and perform services as necessary.
