Key Accountabilities:

Alignment with Vision: Ensure all activities align with the vision of Elara Caring’s board of directors, executive team, and Home Health leadership.

HIPAA Compliance: Maintain patient and staff privacy and confidentiality in accordance with the HIPAA Privacy Final Rule.

Care Verification: Verify the Plan of Care prior to each visit and provide care based on delegation by the RN, physician’s orders, assessment data, and established guidelines.

Patient Care Goals: Incorporate patient care goals from the Plan of Care into nursing visits, documented on skilled nursing notes.

Communication: Report significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other personnel involved in patient care.

Patient Safety: Implement patient safety plans using patient, family, and community resources.

Patient Education: Assist patients in learning appropriate self-care techniques while delivering quality care.

Plan of Care Revisions: Suggest revisions to the Plan of Care in response to patient care issues and notify the RN Case Manager.

Specialized Nursing Services: Provide nursing services requiring substantial skill under the direction of an RN or physician.

Collaboration: Work with the RN Case Manager during normal business hours for advice, assistance, and instructions.

Medication Management: Document and update the medication regimen according to established procedures.

Patient and Caregiver Treatment: Treat patients and caregivers with the highest standard of care.

Hospitalization Prevention: Take all necessary actions to prevent avoidable hospitalizations and ER visits.

Positive Communication: Maintain supportive communication with physicians, patients, families, visitors, and agency personnel.

Respect for Personal Property: Handle the personal property of patients and their families with care.

Additional Duties: Perform other duties and projects as assigned.

What We’re Looking For:

Current LPN License: Must meet state requirements.

Experience: At least 1 year in acute care or an equivalent setting.

CPR Certification: American Heart Association or American Red Cross.

Positive Attitude: Bring energy and enthusiasm to every patient interaction.

Dedication to Quality Care: Commitment to providing the best patient care possible.

Strong Communication Skills: Ability to connect with patients, families, and team members.

Reliable Transportation: Essential for performing job duties.
