This position may be filled by a probation or Promotion of a permanent state employee. This position serves as aLPNMDS NURSEfor Nursing Services Department at Villa Feliciana Medical Complex, an acute and long-term care hospital whose mission is to operate health care programs, which provide medical care and treatment for residents with acute, subacute, and chronic illnesses and/or disabilities resulting from trauma or genetic defects.Under the close supervision and review of a Registered Nurse, the incumbent of this position coordinates facility wide programs, which will include but not be limited to MDS Coordinator, Interdisciplinary Staffing Team & LTC training program.The duties of this position are as follows:Responsible for assessing new admissions, completing assessments (Minimum Data Set) and developing a care plan according to the Resident Assessment Protocol Summary that are individualized for the resident.Responsible for providing a comprehensive assessment (Minimum Data Set) to approximately 80 residents assigned on an annual basis and for any assessed significant changes in the resident's condition.Responsible for coding disease diagnoses identified by the physician in the Minimum Data Set (MDS) on admission, annually and for any significant changes in condition.Responsible for providing a quarterly review on all residents assigned, including new diagnosis and diagnosis codes occurring in the quarter.Develop care plans on all residents on the buildings on an annual basis, as the resident’s condition changes, and update these on a quarterly basis to ensure that treatments and therapies are being carried out.Responsible for compiling discharge tracking and re-entry computer assessments and for maintaining an accurate schedule of assessments and staffing.Responsible for coordinating information from all members of the Treatment Team to formulate the assessments and care plans.Ensure that the Treatment Team is aware of any changes needed in the assessment, care planning or staffing process and coordinating the accuracy and completeness of the MDS.Acts as leader of interdisciplinary team, conducts and documents staffing on all residents on a quarterly basis and as their condition changes.Acting as a patient advocate during staffing to ensure that all of the patient's concerns and requests are addressed.Participate and document special staffing on residents as requested by physician, administration, treatment team, direct care staff or family of the resident.Assist with discharge planning for the patient and follow-up on any readmission for any changes in care.Responsible for providing staff nurses with an awareness of documentation requirements to assure accuracy of MDS for maintaining clinical validation.Responsible for providing in-service training on LTC computer program to all newly hired staff and continuing training as needed to all staff as LTC program changes.Assists with writing/revising policies and procedures related to MDS assessments, LTC program and Staffing requirements.As part of a Career Progression Group, vacancies may be filled from this recruitment as a Practical Nurse License 1, Practical Nurse License 2, or Practical Nurse License 3 depending on the level of experience of the selected applicants(s).
