Position Summary: The Behavioral Health Navigator assists with patients who have complex psychosocial needs and require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Participates in the development of a discharge plan of care for high-risk patient populations. Receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. All age groups are served by this position.Minimum Qualifications:Required
Bachelor’s Degree in Community Health, Medical Social Work, or related field
American Heart Association BLS
Six months minimum Community Resource experience
Preferred
Master of Social Work (non licensed)
Preferred: One year community resource management experience

Collaborates with the Crisis Intervention Specialist or Behavioral Health Specialist to develop and implement an individualized person-centered treatment plan.
Identify needs in social determinants of health and connect the patient and family to community resources
Track high frequency/at risk patients throughout the course of care and update team of potential issues
Advocating for patients and their needs
Helps with patient assistance programs
Assists and set ups Telehealth support for patients
