LPN - Case Manager

LPN Case Manager in North Kansas CItyDescription:

Acts as a liaison/advocate for patients with complex health issues to ensure easy and efficient access to any identified needs

Reaches out by phone to patients on a routine basis to provide ongoing support.

Arranges for any outside services needed: i.e. home health, equipment needs

Reviews medications and addresses any concerns in collaboration with the patient's physician.

Helps patients achieve their personal health and wellness goals.

Addresses urgent calls quickly during office hours

Ensures prompt identification of patients with high risk conditions and determines the appropriate disposition of the issues and/or concerns of the patient

Fills out FMLA and Disability paperwork

no weekends

Pay: $21-35/hour (pay is calculated based on experience)Qualifications:

High School Diploma or GED

Completion of accredited LPN program

Active license in the state of Missouri
