The MA serves the patient and the provider by ensuring comprehension of and compliance with discharge instructions in order to reduce emergence department visits and rehospitalizations, improving post-discharge health outcomes and decrease health care expenditures.Primary Responsibilities:

Obtains relevant information within the Peace Health/Sacred Heart and McKenzie Willamette Medical Center EMR on a daily basis and identifies patients needing follow-up. Obtains medical records from out of area inpatient facilities. Tracks patients discharged to Skilled Nursing Facilities and acquire patient records when discharged from SNF in order to perform TOC medication reconciliation

Maintains current knowledge of disease processes and follows up with patients to reconcile medications and assess the need for further assistance and appointments. Consults with RN, AP or Physician when appropriate

Takes initiative to problem solve and research within the hospital systems and Oregon Medical Group EMR when required and appropriate

Works efficiently within EPIC, charting all patient contact accurately and succinctly within the EMR, maintaining strictest confidentiality

Performs medication reconciliation, documenting changes accurately

Schedules hospital follow up visits as needed complying with organizational goals and patient needs

Responds to incoming phone calls, places outgoing calls and refers if necessary

Identifies patients meeting criteria for Case Management follow up and notifies CM

Performs other duties as assigned

High school diploma or equivalent

MA Certification from the AAMA, NCCT, NHA, or RMA required, or the ability to obtain certification within 60 days of hire

Healthcare provider level CPR/BLS certification

5+ years of Medical Assistant experience

Preferred Qualification:

Experience with electronic medical record systems
