As one of the most technologically advanced hospitals in the world we provide a full range of services including: maternity, cancer and surgery care as well as diagnostics and emergency services.As a Social Worker at CHI Health Lakeside, you provide direct social work services including assessment, treatment planning, intervention, counseling, and recommendations to individual patients (adolescents, adults, elderly) and their families/significant others and/or groups of patients.Responsibilities

Conduct and document psychosocial assessment of patients

Share responsibility in identifying appropriate decision makers if the patient is without capacity, educating all patient’s rights and encouraging participation in decision-making and care

Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs

Identifies mental health needs of patients and provides counselling and referral as indicated

Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community. Informs other members of the health care team about the factors that may affect the patient’s health and well-being

Serves as a resource to patient, family, and healthcare team regarding emotional and behavioral issues and refers to community services available to adolescents, adults, and elders

Remains current of Medicare and Medicaid laws and guidelines, Medicare conditions of participation, managed care contracts, and other legal and regulatory requirements as the relate to healthcare social work services. Assures patients are informed of rights and responsibilities as participants in these programs

QualificationsRequired Education for Staff Job LevelsMaster’s Degree from a Council of Social Work Education accredited School of Social WorkRequired Licensure and CertificationsState of Nebraska or Iowa certification or licensure required.Required Minimum Knowledge, Skills and AbilitiesThree (3) years clinical social work or allied heath practice, with hospital or medical background experience.PREFERRED Qualifications

Creative problem resolution skills, excellent interpersonal and teamwork skills, a positive, professional image, and a highly independent and organized individual, with excellent verbal and written communication skills.

Computer skills required; must be able to document in electronic medical records.

Exercises knowledgeable, independent judgment on routine and non-routine matters.
