Overview: RiverWoods Durham is currently seeking a full time Memory Support Program Coordinator to join the Life Engagement team! The Memory Support Program Coordinator will lead the implementation of recreational, social, intellectual, emotional, and spiritual programs for our Memory Support & Enhanced Support households. Our commitment is to create the possibility for each resident to live their best life throughout every stage of their lives. Responsibilities:

  • Develop, coordinate, and lead programming in Memory Support and Enhanced Support
  • Keep current on economic conditions or developments of new products or services and recommend adjustments in the memory support or enhanced support program that ensure continued ability to provide daily meaningful person-centered activities
  • Serve as a liaison between families and residents
  • Interview with residents and their families upon admission to develop a comprehensive assessment of activity needs
  • Collaborate with the Life Engagement and Community Life teams to share programming ideas and inventory of supplies to provide residents with an enriched and varied monthly calendar
  • Responsible for enhancing the quality of life for all residents by ensuring socialization and therapeutic needs are met in an individualized program that provides meaning and purpose
  • Ability to teach residents and their families about dementia and Alzheimer's disease
Qualifications:
  • Must possess a minimum, associates level college degree or equivalent in experience and skill.
  • Preferred minimum of two (2) years equivalent in dementia/Alzheimer's programming within a long-term care or memory support community
  • Must be willing to obtain certification for Virtual Dementia Tour, CPI Dementia Care Training, Certified Dementia Practitioner (CDP), Music & Memory, and certification through the National Certification Recreations Specialist (CTRS) or an Activities Professional who is certified by the state of New Hampshire
