Degree and scope of service intervention vary based on program as well as the changing needs of the individual. FOR ADOLESCENT PARTIAL CARE, ADULT PARTIAL CARE, AND ACUTE PARTIAL HOSPITAL:Responsible for providing direct care services including individual and group counseling, support and other activities; participating in comprehensive assessment, development of treatment and discharge planning; maintaining clinical documentation, participating in and facilitating family and collateral meetings, team meetings and case conferences to ensure continuity of care.EDUCATION, EXPERIENCE, TRAININGEDUCATION:Masters degree from an accredited college or university in psychology, social work or rehabilitation. Bachelor’s degree candidates currently enrolled in a Graduate Program and with additional experience will be considered on a case by case basis.EXPERIENCE:Two years mental health experience. Bachelors’ level candidates will need 3 years relevant experience with at least 1 year in a mental health case management position.TRAINING:Comprehensive understanding of mental health services and role of case managers in mental health services.
