Assist families in finding the proper resources to supplement medical care and quality of life. Monitor family and patient s understanding and compliance with treatment goals. Interact with social services and community agencies. Support family through provision of resources and support networks as appropriate.Essential Job Functions

Effectively engages individuals|families in helping relationships using acquired professional skills.,

Engages in advocacy for patient, and foster family's abilities to assume independent skills in seeking and obtaining supports and services for child.

Refer patient and families to community resources to provide access to services such as financial assistance, legal aid, housing, job placement or education.

Effectively communicates financial and social needs|barriers|issues to the multidisciplinary team, physicians and external agencies.

Identifies families at high risk for child neglect or abuse and makes appropriate referrals.

Be a resource for families to ease them through transitions and hardships.

Completes proper documentation in a timely manner. Provides necessary documentation to external agencies as needed.

Acts to enhance understanding of CSH services and mission through development of relationships with community and government agencies, service providers and referral sources.

Participates in projects for the department that are consistent with department and hospital goals and missions.

Formal Education and Job-Related Experience

Bachelor's Degree in Social Work. Master's Degree preferred.

Minimum of three years' experience in appropriate clinical setting.

Excellent communication skills, both verbal and written.

Excellent Interpersonal skills.

Good planning and organizational skills.

Certification

Meets the accrediting and|or licensing standards of the profession.

AHA BLS Provider Certification required.
