SupervisorReq #:0000150711Category:LeadershipStatus:Full-TimeShift:DayFacility:Clara Maass Medical CenterDepartment:Arrive Together GrantLocation:Clara Maass Medical Center, 1 CLARA MAASS DRIVE, BELLEVILLE, NJ 07109Job Overview:Under the direction of the Director, the AT Outreach Supervisor participates in the Co-Responder Program (law enforcement officer and screener respond together to emergency service calls and/or follow-up visits that relate to behavioral health crisis) otherwise known as the Arrive Together program within the scope of practice defined by the Screening Law and other regulatory standards. Assures safety in practice by maintaining a realization of their own individual level of knowledge and experience and assumes responsibility for advancing knowledge by communicating with and utilizing resource persons/programs available. The Arrive Together Outreach Coordinator will individualize the plan of care to address a specific patient s needs. Works with law enforcement in identified municipalities and other community agencies, to achieve best service possible to patients. Responsible for supervising and assisting inpatient care issues as well as follow up on assigned tasks and for consistently setting and evaluating priorities.Qualifications:Required:
MSW/MA degree in related field, or BA/BS plus three years of prior experience in a psychiatric setting, one of which is in crisis setting or enrolled in a Master s program with two years of prior experience in a psychiatric setting, one of which is in a crisis setting.

Essential Functions:
Responds to emergency calls along with the law enforcement officer in a professional and timely manner
Demonstrates ability to perform crisis intervention and screening services consistent with the screening statutory and regulatory duties at each call for service.
Demonstrate the ability to collect data relevant to the assessment of the AT program as specified by Clara Maass Medical Center, the Department of Law and Public safety, and/or Department of Human Services, Division of Mental Health and Addiction Services.
Demonstrate the ability to comply with the training provided by the law enforcement agency/police department on the proper procedures and protocols for riding a law enforcement vehicle and/or responding to calls with a police officer prior to participating in the program.
Demonstrates the ability to do follow-up visits with mental health consumers from prior services or initiate contact with individuals identified as at-risk within the designated municipalities.
Coordinates the schedules for the Clara Maass Arrive Together program.
Organizes monthly AT meetings with identified law enforcement departments to troubleshoot issues and identify best practices.
Demonstrates ability to communicate clinical information effectively with the screening psychiatrist
Demonstrates ability to communicate effectively with other members of the health care team, internal and external.
Demonstrates ability to communicate effectively with the Charge, Shift Supervisor, other as pertains to the patient s treatment plan and disposition
Demonstrates ability to conduct a thorough ongoing assessment according to departmental and DMHAS guidelines
Demonstrates professional behavior according to hospital codes in section Conduct for Patients/Customers in the Employee Handbook.
Describes Screening process and updates progress of evaluation/interventions to patient/family
Documentation on Presenting Illness indicates concept of Why patient is here today
Documentation indicates attempts made to provide clinical information and obtain insurance pre-certification
Documentation indicates collaboration with patient and/or family in the development of patient care planning and discharge
Documentation indicates gathering of relevant collateral information
Documentation indicates that patient understands the concept of voluntary admission
Documentation indicates that patient/family were provided with information based on Discharge Plan/Teaching Checklist
Documentation indicates thorough completion of request for off-site screening evaluation
Documentation indicates thorough completion of requests to do community outreach
Documentation reflects clinical decision of Outreach outcome based on criteria as per Screening Law
Evaluates effects of interventions throughout the patients stay and updates the plan of care as per hospital policy.
Prepares and maintains accurate records and reports in a timely manner prior to end of shift.
Provides hotline information/referral/interventions that conforms to Screening guidelines
Screening Assessment indicates determination of most appropriate services/ disposition according to patient s clinical needs consideration of least restrictive alternatives.
Screening Document contains clear description of dangerousness due to mental illness as per Screening Law and guidelines from DMHS Screener Certification training
Screening Document reflects clear description of patient s mental illness as per Screening Law and guidelines from DMHS Certification training.
