Position Overview:

We are seeking an experienced Support Coordinator (1099 Independent Contractor) to join our growing agency. This role offers a flexible schedule, independent caseload management, and compensation based on experience and assigned individuals.

Responsibilities:

Develop, revise, and maintain Person-Centered Individualized Service Plans (ISPs)

Conduct required monthly monitoring and visits

Maintain compliant documentation meeting DDD standards

Assist individuals and families with service selection and coordination

Advocate for individuals’ goals and rights

Complete required DDD trainings

Qualifications:

Bachelor’s degree or higher (DDD requirement)

Minimum 1 year experience working with individuals with I/DD

ISP writing and DDD system experience

Strong organizational and communication skills

Reliable transportation

Ability to work independently

Ability to pass background checks

Compensation (1099 Contractor):

Competitive PMPM distribution rate

Earnings based on caseload size

Flexible schedule

No micromanagement or excessive caseloads
