Case Manager

An Assisted Living Facility in Canarsie, Brooklyn is looking for a Case Manager. Must have 2 years of Case Management experience.

Case management duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the Geriatric population's health and human services needed.

Full Time position, $23.00 per hour to start, health insurance, paid time off, plus benefits of job.
