CDPAS CoordinatorFarmingdale, NY 11735, USA ● Jamaica, Queens, NY, USA ● Staten Island, NY, USA Req #12112 Thursday, October 31, 2024Centers Plan for Healthy Living's goal is to create the ultimate healthcare experience that provides our members, their families, healthcare decision makers, and general caregivers with the guidance and plans they need for healthy living.JOB SUMMARY:The Intake CDPAS Coordinator (in-house) is responsible for monitoring CDPAS events to ensure accurate and timely completion; outreach to MDs for new and updated MD DOH forms; confirmation of PA hires with fiscal intermediaries and other communication needed with vendors; and telephonic CDPAS education to prospects and members interested in or receiving CDPAS.PRIMARY RESPONSIBILITIES:

Telephonic outreach to prospects and members interested in or receiving CDPAS.

Receive, review, and upload CDPAS documents into member files.

Communicate with CPHL CDPAS vendors regularly and as needed.

Monitor, review, and complete CDPAS events within membership database, handling a high volume of requests accurately and efficiently.

Accurately and clearly document all tasks and outreach efforts, maintaining good records.

Answer a high volume of emails in a timely and professional manner.

Other assignments and duties as needed.

Education and Experience:

EducationRequired:

HS diploma

Excellent written and verbal communication skills

Proficiency in the use of Microsoft Office tools

Preferred:

Associates or Bachelorette degree

Type of Experience Required:

Customer service/ excellent phone skills

Preferred:

CDPAS

Understanding of and sensitivity to member’s cognitive ability, means of communication and cultural differences and needs of the community are essential.

Strong problem-solving ability.

Specific Technical SkillsRequired:

Computer and Excel proficiency
