Certified HHAs - Genesee County residents need you! Starting rate is $17.55/hr. plus additional incentive pay available Summary: A Certified Home Health Aide provide personal care and supportive services in the patient's home.Essential Responsibilities:

Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.

Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.

Immediately report changes in client’s condition to supervising professional.

Ensure the safety of the patient, patient’s family and/or the employee.

Maintain HIPPA.

Complete and submit necessary patient documentation including charting tasks and observations on android device.

Attend ongoing in-service programs in a timely manner to maintain certification.

All other duties as assigned.

Minimum Qualifications:

High School diploma or GED preferred.

Completion of an approved program of PCA or HHA certification as designated by NYS Dept of Health.

Must have proof of completion of HHA Training Program (Original HHA Certificate).

Must have worked for a Licensed Home Care Agency (LHCSA) within the last 24 months as documented on the NYS Home Care Registry

Valid Driver's License and reliable transportation.
