The Community Nutrition Educator a hybrid non-exempt position paying $29 per hour at 35 hours per week. Flexibility to work from 2-3 days a week. Primary Purpose of Job:
The Community Nutrition Educator is responsible for nutritional health education to clients with HIV and related program responsibilities.
Principal Duties & Responsibilities:
Develop nutrition health education plans that incorporate: goals and objectives for HIV+ clients and provide group and individual nutrition health education to improve retention in medical care and treatment.
Establish connections and collaborate with Medical Nutrition Program/Practitioners and Registered Dietitians.
Complete screening & rescreening forms and document eligibility including HIV status & homebound status.
Maintain client records and electronically document attendance, participation, progress in meeting goals, and track referrals.
Ensure client adherence to Nutrition Health Education and compliance with food and meal eligibility criteria.
Act as Team Leader with Nutrition Program Assistant in conducting program outreach and coordination with medical providers.
Attends AIDS Institute meetings and trainings.
Monitor food-related activities including ordering food & maintaining food inventory and the distribution of weekly pantry bags to clients.
Coordinate annual HIV confidentiality training for staff and volunteers.
Supervises drivers and volunteers.
Knowledge, Skills, & Abilities Required:
Registered Dietetic Technician or BA or BS required in Nutrition or a related field such as health education or family and consumer sciences.
Minimum 2 years of experience working in the field of HIV/AIDS or with other chronic illness required.
Must be familiar with community services for Persons Living with HIV/AIDS in Nassau County and have the ability to work cooperatively with other providers.
Good communication and writing skills.
Must be able to lift a minimum of 10-15 lbs.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
Must possess valid NY State Drivers License, have access to an automobile.
Full-time
