Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
Communicates patient information to referring providers and appropriate staff in a timely manner.
Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.

Job Qualification
Bachelor's Degree in Food, Nutrition or Dietetics, required.
Registered Dietitian, required.
Current Diabetes Educator Certification, required.
Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
OR
Bachelor's Degree in Nursing, required.
Current license to practice as a Registered Professional Nurse in New York State, required.
Current Diabetes Educator Certification, required.
Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
