Req Number 146779FlexStaff is seeking a Home Care Nursing Manager to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. The area of responsibility will be in Westchester, NY.Must have a NYS RN License. Additionally, an ideal candidate would have a nursing degree, and completed NYSED-approved infection control coursework; minimum of 2 years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic; Minimum of 1-year experience working with the elderlyThis is a permanent, full-time job. Schedule: Monday - Friday 8:30 AM - 5:00 PM, 37.5 hours per week.In this role you will be responsible for overseeing the day-to-day operationsResponsibilities include, but not limited to:
Oversee development and execution of personalized care plans to enhance participant satisfaction and outcomes.
Facilitate IDT meetings and SDR processes, completing necessary attestations and collaborating with Virtual IDT as needed.
Serve as on-call manager during non-operational hours, handling follow-ups and initiating grievances timely.
Generate Clinical Delivery reports and represent CHN in IDT meetings.
Balance staffing and resources for seamless team operations and ensure communication with IDT and related parties for coordinated care.
Work with clinicians to ensure accurate assessments using UAS and UAS analyzer.
Investigate incidents and ensure timely Level 2 documentation per CLHC regulations.
Ensure complete and accurate documentation for participants, including medical records.
Review participant charts for quality and coordinate care management with the Transitions in Care Team.
Arrange long-term care services as needed and monitor care management effectiveness.
Maintain compliance with CLHC standards and regulations, staying informed on health-related practices.
Recommend service and process improvements and collaborate with HR on staffing, training, and regulatory compliance.
Evaluate and support direct reports, providing feedback and conducting performance reviews.
Resolve employee grievances and serve as Director of Patient Services for the LHCSA as assigned.
Perform additional duties as needed.
