Performs a variety of functions and is a single source of contact for members, health care personnel and all other entities involved in managing care.

Identifies alternate treatment programs.

Consults with physicians, providers, members, and other resources to evaluate options and services required to meet an individual's health needs.

Promotes quality and cost effective outcomes; serves as liaison to physicians and members.

Determines member assignment of treatment, and pre-admission/post-discharge counseling for an acute condition.

Establishes relationship with the member.

Handles immediate post discharge follow-up or until all short term care needs are met.

Provides education/local resource information and encourage member (self) education.

Job Qualification

Bachelor's Degree in Nursing (BSN) required.

Current License to practice as a Registered Professional Nurse in New York State required. Certified and/or Licensed as a Massage Therapist, Holistic Nurse or Reflexology Practitioner, required.,

4-7 years of technical experience and 0-2 years of leadership / management experience, required.
