The South Glens Falls CSD has an anticipated vacancy for a LOA School Social Worker at Moreau Elementary to begin on or about January 5, 2025 to March 17, 2025.Performance Responsibilities:
Consults with teachers and other school personnel regarding student
Consults with appropriate people to assist students in improving conditions which impact behavior, social adjustment and performance in school.
Provides direct counseling services to students and parents as needed to include home visits whenever appropriate and applicable.
Assesses student’s home and neighborhood environment as individual circumstances necessitate.
Serves as District liaison with community service agencies.
Prepares and assists parents and/or students for referral to outside agencies.
Completes all reports that are required for proper intervention in a timely and accountable manner.
Attends CSE, CPSE, IST, 504, staff and other meetings when requested.
Seeks support (advice) from other behavioral specialists (psychologist, psychiatrist) with appropriate and applicable cases.
Maintains certification in crisis management appropriate with program (i.e. TCIS)
Maintains records on a consistent and accurate basis.
Establishes an on-going parent and public program on a monthly basis.
Provides individual and group therapeutic counseling to students and their families.
Provides crisis intervention services.
Provides social work case management for students and families.
Conducts staff development on issues related to social-emotional-environmental factors that impact learning.
Conducts classroom meetings, psycho-educational social skills groups and classroom presentations on identified areas of concern for the students.
Maintains required clinical records to include Medicaid reporting and submits appropriate documents for statistical reports with adherence to program standards in school social work.
Conducts home visits related to establishing communication and positive connections between the parent/guardian and school setting around identified issues.
Completes functional behavior assessments on identified students as needed.
Assists IST Team in developing behavior management improvement plans (BIP) and Individual Crisis Management Plans (ICMP) as needed.
Direct support to 8:1:2 classrooms and assists in tracking student data with these classes.
Completes classroom visitations to observe students.
Any other such duties as assigned by the Superintendent of Schools.
Job QualificationsQualifications:
Master’s Degree from an accredited College with Social Work program.
Requires New York State Social Worker Certification
Other as determined by the Board of Education and Superintendent of Schools
