Growing together.The LVN Case Manager will perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). Current unrestricted NY LVN license
2+ years of experience in a physician’s office, clinical or hospital setting
Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
Proficient computer skills to work efficiently with electronic medical records
Ability to travel within a commutable distance of Poughkeepsie for internal and external meetings
Preferred Qualifications:
Experience related to patient education and/or motivational interviewing skills and self-management goal setting
Case management experience
Comfortable speaking and presenting to hospital partners and executive leadership
New York Residents Only: The hourly range for this role is $19.47 to $38.08 per hour.
