Summary of Need: As a pharmacist, you would be responsible for the provision of pharmaceutical care, including the reviewing, monitoring, dispensing, and distribution of medications.Job Responsibilities include, but are not limited to:Candidate will process orders for both medical and psych providers, do discharge consults, be on hand as a resource, for patients, nurses and providers. You would also be responsible for providing drug information, conducting medication counseling sessions, supervising pharmacy aides, and participating in pharmacy and facility wide performance improvement activities.You may be required to provide clinical and distributive services including Drug Utilization Reviews (DUR), pharmacotherapy consults, anticoagulation management, health and wellness program participation, dispensing and other clinical and distributive services as assigned.You would optimize medication use by actively managing drug therapy and by identifying, preventing, and resolving medication-related problems either directly or as part of the treatment team.You would collect, analyze, and report on medication therapy related data to enhance patient safety and improve patient care and be responsible for maintaining safe, effective and efficient operations in the pharmacy department.You would also be involved in maintaining accurate dispensing, distribution and related inventory records. Requirements: New York State Licensed PharmacistPrefer someone with long-term care experience, with a minimum of 1 year experience.
