What you will do:
Work with an interdisciplinary team to assist patients and their families in dietary needs for optimal health.
Responsibilities
Work one on one with patients and families in their home setting or per televisit which allows you to customize their diets.
Educate patients (and their families) on specialized diet and assist in weight management
You may have the opportunity to work with MDs and outside agencies for pts with enteral feeding and TPN.
Patients are referred following a hospitalization, inpatient rehab stay or by their physicians.
What you will need:
Bachelor Degree in Nutrition/Dietetics (Didactic Program for Dietetics).
Registered Dietitian Certification with New York State is preferred. ADA registration preferred but not required.
Minimum of 1 year Dietitian experience preferably in an acute setting preferred
Home care experience preferred but not required
