Req Number 141421Job DescriptionCoordinates and guides all phases of the referral, intake and admissions processes to ensure timely and appropriate admissions of rehabilitation outpatients. Coordinates and monitors the department master therapy schedule.Job Responsibility

Monitors the flow of patients being admitted to the facility; intervenes when delays are identified related to admission paperwork, bed turnover and patient transport.

Interviews residents/significant others to obtain all relevant demographic and insurance data; oversees team members while they obtain necessary signatures on required forms and enter all pre-admission and admission information into department computer system.

Provides interpretation of regulations to residents (i.e., visiting hours, resident services, Bill of Rights, etc.).

Monitors and ensures completed charts accompany patients upon admission.

Serves as a liaison between admitting, nursing stations, patient transport, and medical records to facilitate optimal patient flow.

Responds to patient/family complaints as they relate to the admissions and discharge processes.

Recommends and monitors approved work-flow changes.

Participates in developing and delivering training related to operations.

Job Qualification

Associate's Degree required, or equivalent combination of education and related experience.

3-5 years of relevant experience, required.
