GENERAL PURPOSE:The social worker assigned to Pediatric Behavioral Health & Wellness Outpatient Services and functions as a primary therapist providing psychosocial assessment and social work and behavioral health treatment for child and adolescent patients with mental health diagnosis and their families. The social worker also provides crisis and risk management interventions, collaborates with collateral contacts, and coordinates linkages to necessary community resources. The social worker serves as a discipline representative on one or more of the clinic’s multidisciplinary diagnostic and treatment teams and assumes responsibility for patient screening, as delegated by the Intake Committee.RESPONSIBILITIES:

Provides direct evidence-informed clinical treatment to assigned patients, including individual and family therapy; as well as crisis intervention; assesses successful completion of therapeutic goals (i.e., progress monitoring) and plans patient discharge accordingly; meets minimum target for clinical productivity set by service director.

Under general direction and with considerable latitude for independent judgment performs complex psychosocial assessments and diagnostic formulations for patients and their families, to include child abuse/neglect assessment and reporting as indicated. Provides back-up social work services as requested during the absence of co-workers.

Documents treatment in patients’ electronic medical record in compliance with Program, Department and Hospital regulations. This includes the development of family centered, comprehensive and individualized treatment plans for patients with clearly defined therapeutic goals with patients and families; ensures that Social Work Division, Department, Program, and Hospital policies and procedures are implemented with regard to professional practice, patient care and discharge planning

Provides group therapy services to assigned patients

Establishes and regularly maintains cooperative working relationships with community and health care agencies, as well as professional and administrative staff of the Medical Center and University. Coordinates with support staff regarding patient scheduling, fees, and insurance issues.

Collaborates with referral sources and other professionals both internal and external throughout patient care activities (e.g., PCPs). Mobilizes family and natural support networks when clinically indicated.

Participates in routine committee process of the Social Work Division and the Child and Adolescent Clinic, and complies with Social Work Division and Departmental quality assurance and staff development requirements; participates in activities of the Social Work Division including, but not limited to timely completion of statistical reports/forms, time sheets and other Hospital or Division forms, Hospital and Division mandatory in-service training programs and UHS reviews, and Social Work Division and Program Staff Meetings; maintains professional CEU requirements by reading, attending seminars, workshops and conferences.

Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences; participates in available cultural competence events and training appropriate to job duties.

Teaching: Based on experience and as determined by the social work supervisor, the social worker may supervise social work or other master’s level graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.

Research: Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.

Other duties as assignedQUALIFICATIONS:

Master’s degree from an accredited school of social work (CSWE) required.

2-3 years professional social work practice beyond the Master's degree preferred.

Prior experience working in a psychiatric care setting and working with pediatric and adolescent population preferred.

Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions preferred;

Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families preferred;

Dedication to delivering a team-based integrated model of care preferred.

Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome preferred.

Master's degree from an accredited school of social work. Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
