Role Summary: Under the direction of the Clinic Manager, he/she shall work as a member of a strong team to provide nursing care for the comfort and wellbeing of all participants. Triages participants; performs medication reconciliations on regular basis in coordination with Home Care Coordinator; administers medications; performs initial, semi-annual, and annual assessments; assist MD with ordering of: medications, labs, procedures, consultation visits; changes dressings and provides wound care; serves as liaison, between providers, participants, staff, and families. Documents care delivery, patient condition, reaction and progress. Utilizes the Nursing Process (Assessment, Planning, Implementation and Evaluation), in the delivery of professional nursing care to the clinic population in accordance with physician practices policies and procedures. Provides patient education, especially in regard to pharmaceuticals and supports the business goals of the practice. He/She will also be responsible for being a licensed member of our 'On-Call' team.

Required Education: Bachelor of Science in Nursing. Effective 3/1/18 Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position. Previous signed BSN commitment letter OR grandfathering will be accepted between departments and Mission Health entities. Exemptions may be approved with CNO System Council approval.

Preferred Education: Masters in Nursing.Required License/

Certifications: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBONCertificate: BLS Healthcare Provider; HeartsaverPreferred License/

Certifications: National Certification in area of specialty

Required Experience: Minimum of 1 year working with the frail or elderly.

Preferred Experience: Minimum of 3 years of nursing management in a community home based setting preferred.
