Description The Beacon Program is seeking a full time licensed Social Worker to work Monday-Friday (day shift) including one weekend of call per month. LCSW or LCSWA is required for this role.The Beacon Program provides comprehensive, specialized clinical social work services to adult patients, family members, and employees who have experienced some form of intimate partner and/or family violence; including human trafficking and vulnerable adult abuse. Services provided include bio-psychosocial assessments, advocacy, danger assessments and patient/family/employee education. Provides counseling, safety planning and obtaining community assistance by referral to proper resources relating to violence and abuse. Obtains, interprets, and communicates to staff and physicians necessary information regarding patients and families, including bio-psychosocial history information. Identifies possible or actual patient/family crises, providing crisis intervention and emotional support, and creating and facilitating effective plans for resolution. Develops and maintains good working relationships with outside community health and social agencies. Reaches out to the community to learn of and utilize new resources for meeting patient needs. Assists patients and families with obtaining community assistance by referral to proper resources. Documents group therapy and other care activity in patient charts according to department policies and procedures.
