The Case Manager, MSW integrates the duties as case manager by performing discharge planning evaluation of assigned patients. The Case Manager, MSW/BSW is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal/external resources. MSW/BSW functions as a primary contact for coordination of internal and external resource services to facilitate care delivery in the discharge plan. As a member of the healthcare team they communicate effectively with the interdisciplinary team to ensure safe, quality care is provided to all patients. Minimum Education
