OverviewCHI St. Alexius Health is seeking an Advanced Practice Provider , NP or PA, who is passionate about Diabetes Management to work with established patients in Bismarck, ND! Join this highly collaborative, inter-professional, dedicated team which is charged with improving care for patients with diabetes, particularly with respect to transitions of care from the IP setting.Practice Details :

Outpatient, clinical position providing care for patients with diabetes

Monday - Friday schedule; 8:00 am - 5:00 pm; No Call

EPIC EMR

Built-in referral base from within CHI St. Alexius

Provide routine diagnostic, therapeutic and emergency care for patients

Maintain a caseload of established patients

Interpret and integrate data in order to appropriately diagnostic and provide therapeutic procedures as needed

Perform complete physical exams and record findings

Manage stable chronic disease and refer patients with illness to the appropriate specialty

Interview and advise patients & family regarding treatment, health, and illness prevention as well as recommending community resources to meet patient & family needs

Maintain and review patient records, charts and other pertinent information

Requirements :

Possess excellent interpersonal and leadership skills

Ability to provide exceptional, high quality care

Support a team-based care delivery model

Be ready to hit the ground running and continue to grow this busy endocrinology practice

Preference for candidates with backgrounds in endocrinology, family medicine, or internal medicine

License/Certification :

Accredited Nurse Practitioner or Physician Assistant

Board certified with ability to obtain an unrestricted ND license to practice

CPR certification

AANP certification

ANCC certification

BCLS

Diagnose and treat various medical conditions.

Qualifications

American Board Certified or Board Eligible in Specified Area of Medical Specialty

A valid and unrestricted state medical license or license-eligible
