STATUS: FULL TIME (RF Status -80 Hours/Pay Period)SHIFT ROTATION: Scheduled work days are variable: 12 hour shifts,Weekends and holidays have a variable scheduleDUTIES AND RESPONSIBILITIESAssesses patient and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
Assesses patient and family's financial, social, emotional, educational and environmental resources and identifies needs based on patient's current medical condition.
Develops an individualized plan of care based on the findings of the assessment and needs of the patient and family/caregiver as related to the patients’ medical condition.
Participates in Discharge planning activities for complex patients in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.
Other duties as assigned.

Possesses knowledge of and practices in accordance with the NASW Code of Ethics.
Certifications:Licensed Social Worker (LSW) in the current State.
Social Worker or Case Management Certification (CCM) preferred within one year of eligibility.
Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire.
