Our hospitals, care sites and clinicians are recognized for clinical and operational excellence.General Summary:The Hospital Social Worker provides direct service to patients and their families to promote optimal psychosocial functioning, increase coping with illness and assist with appropriate post hospitalization care, based on age and disability. In collaboration with Care Coordinators, social workers assist patients along their continuum of care. Referrals and inquiries from the community are given assistance with information and resources as requested.To provide the highest quality of care at a reasonable cost in accordance with the rules and regulations of federal, state, agency and the National Association of Social Work Code of Ethics.To demonstrate the philosophy, mission, and values of the hospital during all interactions with patients, families, physicians, staff, and other customers with the intent of furnishing the highest satisfaction to these individuals.Minimum Knowledge, Skills and Abilities:Knowledge and application of social work theory and practice, including counseling, and psychosocial assessment at a level normally acquired through completion of a Bachelor’s degree in Social Work.One or two years experience in an acute care hospital, working with medical/surgical, OB and pediatric patients. Knowledge of age specific competencies and internal and external resources available to these patients.Computer literate (Windows environment).
