Establishes internal and external outreach referral plan for all service lines. Will actively participate in referral process with patients. Continues to determine opportunities for improvement by collecting and presenting data and meeting with the leadership team. Processes patients from referral through discharge including working with insurance companies, referring and admitting physicians and discharge planning.Responsibilities:Establish internal and external outreach plan for all service lines. Follow-through with outreach plan in order to achieve progress and make changes as needed. Present plan to hospital leadership, review needs for success as well as roadblocks.

Coordinate meetings, educational events, Grand Rounds and related activities in order for referral sources to learn more about our services and increase the number and quality of referrals. Actively participate in referral process of inpatients including meeting the patient and family at referring location as well as the referring team, complete IAAs in Epic, and educate CCCHR caregivers on specific clinical needs.

Report referral data and present opportunities for improvement.

Troubleshoot on the spot when at a referring location, represent hospital professionally in clinical business environments, and consistently find opportunities to outreach CCCHR services including hospitals, private practices and payers.

Accepts admission referrals for Inpatient and Day Therapy Program through multiple communication lines including phone, fax and email.

Completes the IAA (Individual Admission Assessment) for each patient by visiting the patient and parents, meeting with the clinical team and any other people who can assist in getting to know the patient in order to prepare and provide exceptional care.

Attends NICU, PICU and other Rounds at all Cleveland Clinic Children's Hospitals in order to discuss potential patients and ensure that referral sources continue to be aware of the care, treatment and services at Children's Hospital for Rehab

Communicates patient information to caregivers and teams in order to be determine appropriateness and be fully-prepared for all needs of the patient prior to admission including provider and insurance company discussions.

Register patient into the EMR and hospital data collection portals including details throughout the admission and discharge process. Ensure consent is received and entered into the EMR prior to admission.

As part of the process to educate patients and families on their (potential) admission, works with the family and patient including touring the hospital.

Leads and reports out on the daily Bed Huddle in order to facilitate teamwork for every patient.

Other duties as assigned.

Education:Minimum of Bachelor's Degree in Health Science related field, preferably nursing.

Certifications:Currently licensed as a Registered Nurse and/or Advanced Practice Registered Nurse in applicable state OR as a Physician Assistant through the NCCPA (or successor) OR Registered Nurse (RN) OR as a Physical Therapist (PT) or Occupational Therapist (OT) or Speech Language Therapist.

Complexity of Work:Within the scope of the job, requires critical thinking skills, decisive and clinical judgment and the ability to work with minimal supervision.

Work Experience:3-5 years clinical experience.

Knowledge of rehabilitation field and processes, pediatrics, developmental milestones.

Experience in Case Management a plus.

Strong clinical assessment skills.

Physical Requirements:Ability to walk/stand for long periods of time.

Carry up to 20lbs.

Personal Protective Equipment:Standard PPE as required by the hospital or referring physician's office.
