As an Aging Care Manager 3, you will be responsible for receiving and investigating reports of neglect and abuse of older adults, petitioning court for intervention, testifying in court, and performing intensive care management duties for high risk individuals. Work includes exercising independent judgment to achieve objectives as well as completing documentation in the state database(s) within the appropriate timeframe. Specialized duties include maintaining a caseload of consumers, responding to client emergencies, and regularly coordinating services with physicians, community agencies, financial institutions, hospitals, nursing homes, and personal care homes.Work Schedule and Additional Information:
Full-time employment
Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
Occasional on-call hours will be required, upon successful completion of training.
Some out of county, overnight travel may be required.Salary:In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONS
Minimum Experience and Training Requirements:
One year as an Aging Care Manager 2 or Aging Program Assessor; or
One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.Other Requirements:

Hiring preference for this vacancy may be given to candidates who live within Schuylkill County. If no eligible candidates who live within Schuylkill County apply for this position, candidates who reside in other counties may be considered.
