UPMC Community Medicine Incorporated is hiring a full-time Clinical Social Worker, MSW to join their team! This position works from 8:00 am to 4:30 pm, with travel between two locations. Monday, Wednesday, and Thursday will be located at our St. Margaret office, and Tuesday and Friday will be at our Oakmont office. A main goal of this social work position is preventing ER visits in this vulnerable population. In this role you will identify ways to cope with daily ADLS, depression, anxiety, emotional and behavioral symptoms as well as primary care giver support.This is a great opportunity to work with the geriatric population and their family members. We are a teaching site, so you will interact with 6 physicians, 3 fellows, 2 pharmacists and resident pharmacists, and a psychiatrist and PA psychiatrist.Apply today and be a part of this great team!Purpose:The Clinical Social Worker is responsible for providing complete psychosocial assessments, crisis intervention, individual, family, group, bereavement and end of life counseling, and timely and appropriate discharge planning based on known or anticipated needs.Responsibilities:

Monitor and evaluates plan, options and available services to effectively meet the patients' needs. Reassess the plan of care as status changes at a minimum of every 1-3 days.

Demonstrate appropriate professional practice, maintaining respect for confidentiality and freedom of choice as outlined by the Code of Ethics of the National Association of Social Workers.

Demonstrate a knowledge of reimbursement systems, the ability to educate patients and families regarding payer requirements/coverage for post-hospital services and the ability to effectively advocate on behalf of the patient to obtain authorization.

Demonstrate the ability to identify, assess, counsel, and effectively intervene with medically and psychosocially complex patients.

Demonstrate expertise in accessing social systems by providing resource-specific information to patients/families, initiating contact with resources and facilitating the patient's/family's ability to accept referrals.

Maintain extensive knowledge of federal, state and local assistance programs and community resources that affect patient needs.

Utilize the modalities of crisis intervention, brief goal directed therapy or other therapeutic counseling methods.

Provide accurate, timely, and appropriate documentation of all social work interventions in the medical record per regulatory policies and procedures.

Complete a psychosocial assessment that includes social, economic, cultural, and behavioral factors, history of child, elder or domestic abuse , prior psychiatric or substance abuse treatment, and all other available resources.

Follow the guidelines of the State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.

Collaborate with the healthcare team patients and families and relevant community resources to develop and implement an individualized plan of care.

Educate and advocate for patients and families to enhance their ability to make informed decisions regarding medical care, discharge planning, advance directives and end of life care.

Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education.

Minimum of two (2) years experience preferred, preferably in an acute care hospital or other health care entity. Licensure, Certifications, and Clearances:

Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
