RN Case Manager - Einstein Montgomery

Responsibilities

• Responsible for pre-authorization and management of all acute patients and members entering EHN system
• Monitors and coordinates care delivery with attending or primary care physician and facilitate discharge planning

• RN licensure from the state of Pennsylvania, required
• One to three years of clinical experience required
• One to two years care management experience preferred
• General knowledge of utilization management within a hospital or managed care organization is required
• Knowledge of managed care contracting preferred
• Excellent communication and customer service skills required
• Working knowledge of PC applications required
