Care Coordinator

Job DescriptionWe are seeking a dedicated, compassionate and highly organised Care Coordinator to join our dynamic team in Hayling Island, United Kingdom. You will play a central role in coordinating our Care Professional team and ensuring that clients receive consistent, high-quality, person-centred care. The role requires excellent organisation, communication and problem-solving skills, particularly in responding to real-time changes in client needs and Care Professional availability.Key ResponsibilitiesLead and manage the Care Professional rota, ensuring all client visits are appropriately and consistently covered. Balance client needs, continuity of care, Care Professional availability, skills and workloads to create an effective and sustainable schedule.Manage the rota in real time, responding promptly to sickness, cancellations, changes in Care Professional availability and changing client needs, while minimising disruption and maintaining continuity and quality of care.Coordinate and optimise care plans, ensuring care remains person-centred, responsive and aligned with each client's individual needs and preferences.Monitor client needs and care delivery, identifying emerging concerns and working closely with the Care Manager, clients, families and Care Professionals to make appropriate adjustments.Maintain accurate and up-to-date records, including care plans, schedules, client information and other documentation, ensuring compliance with company policies and regulatory requirements.Communicate effectively with clients, families and Care Professionals, ensuring changes to schedules, care requirements and other important information are communicated clearly and promptly.Support quality, compliance and continuous improvement, contributing to safeguarding, regulatory requirements and initiatives that strengthen the quality and consistency of care.Provide administrative and on-call support, including reporting, data entry and responding to urgent coordination needs to help ensure uninterrupted care delivery.
