LPN/RN for Home Health Assessments

In Home Care seeks Nurse that will be responsible for going to clients home and completing assessments. This role is location and may require travel and somewhat flexible based upon nurse availability. No prior experience necessary, if you have Home Health experience that would be ideal.

Home Health Assessments

An in-home care assessment is a crucial process that helps determine the specific types of home care services required by an individual. Following the assessment, a Nurse designs the individual Care Plan, which outlines the daily tasks for each home care client based on their unique needs.
