Department Details The 21-bed unit is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and is focused on returning patients to their community. Patients who are admitted to the unit range in age from adolescents to older adults. Patients can be admitted from any unit in the hospital, including neurology, pulmonology, surgery, postpartum, nephrology and cardiology as well as through direct admission from external referrals. This allows the rehabilitation nurse to care for a wide variety of patient populations. Depending on the individual needs of the patient, an intensive rehabilitation treatment plan is established by an interdisciplinary team of professionals. The rehabilitation team includes physiatrists (physicians specializing in rehabilitation medicine), therapeutic recreation specialists, registered nurses, and speech, occupational and physical therapists. Unit staff learn from, interact with and collaborate with health care professionals throughout the hospital to ensure safety and a great patient experience. Patients must have medical needs requiring 24/7 inpatient nursing and the close supervision of the rehabilitation physician. The team addresses functional deficits through treatment interventions, carry-over of tasks and education. Success is measures by how much patients improve during their stay and by how well they are able to function once they return home. Our average patient length of stay is 14 days. In the event that a patient requires another level of care, resources within the hospital are immediately available.

Responsibilities
  • Plans and coordinates patient care, assessment, education, triage and various other nursing interventions
  • Collaborates with colleagues, including physicians, to plan, implement and evaluate care
  • Empathetically cares for patients during all stages of preventive services, health maintenance, diagnosis, treatment and follow-up along the continuum of care
  • Demonstrates competency and uses comprehensive nursing expertise

Qualifications Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor s Degree in nursing preferred. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
