Adoration Health is seeking a dedicated Licensed Practical Nurse for a private duty position in Munford. This role involves providing in-home care for a 47-year-old female patient with trach, g-tube, and colostomy care needs.Key Responsibilities:
Deliver compassionate and professional nursing care in the patient's home.
Manage and monitor trach, g-tube, and colostomy care.
Administer medications and treatments as prescribed.
Collaborate with the healthcare team to ensure comprehensive patient care.
Maintain accurate and detailed patient records.
Provide emotional support and comfort to the patient and their family.

Day Shifts: We have 2 openings for day shifts.

Qualifications:
Current and valid LPN license.
Experience with trach, g-tube, and colostomy care preferred.
Strong clinical skills and attention to detail.
Excellent communication and interpersonal skills.
Compassionate, patient, and dedicated to providing the highest level of care.
Ability to work independently and as part of a healthcare team.

Responsibilities
Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
