Description of Duties (primary & secondary) % of Total Work Effort Data Driven Quality & Performance Improvement Efforts: Create template forEmergency Department (ED) Utilization using claims data. Perform quarterly track/trendingof ED Utilization. Support efforts regarding the administration (including samplingmethodology and stratification approach) of National Core Indicators (NCI) and NationalCore Indicators Aging and Disabled (NCI-AD) and use of findings for longitudinal analysis,quality of life analysis and other efforts regarding use of survey findings for qualityimprovement. Contribute to the drafting of baseline data plans, methodologies,implications, and impact MLTSS evaluation design. Support efforts to complete all quarterly,annual, and interim reporting related to the Evaluation Design. Complete quarterlyEmployment Data Sheet (EDS) reconciliation and develop quarterly reporting template thatsummarizes EDS data for metrics and measuring purposes.60%Health Plan Accreditation & Measures: Review annual reports and measures to gleanfrom and support quality improvement efforts. 10%Home & Community Based Settings (HCBS) Federal Rule/Person Centered Planning(PCP): Oversee Individual Experience Assessment (IEA) data collection (thru formstack),perform analysis, support implementation of quarterly HCBS reporting, utilize HCBSquarterly reporting to develop quarterly metrics for compliance monitoring. Support alignedPerson Centered Support Plan (PCSP) data collection efforts throughout development andimplementation.10%Quality Monitoring (QM): Assist in report template development, finalization, andimplementation. Perform SWOT matrix and analysis, track and trend quality focusedoutcomes utilizing various stratification to drive and inform policy/protocol expectationsand contract/interagency agreement language.10%Reportable Event Management (REM): Perform rigorous oversight of data collectionregarding REM, utilizing program level tracking and trending activities. Aggregate andanalyze data across MCOs and providers to inform system wide improvement efforts.5%Comprehensive Quality Strategy Identify quality metrics across Long Term Services andSupports (LTSS) programs using existing tools and data collection resources that supports the Division s overall approach to quality including incorporation of key systemstransformation goals related to quality assurance and continuous quality improvement 5%
