Description We are seeking a dedicated Associate Patient Care Coordinator (PCC) to join our team. This role involves providing key support functions to ensure quality patient experiences, with a focus on scheduling annual screenings, verifying insurance, and handling follow-up communications.Essential Job Functions:

Conduct telephonic outreach to patients to schedule annual screenings for PSU health events.

Collect and verify demographic and insurance information, including eligibility, co-insurance details, language, and preferred contact methods.

Make timely reminder calls for upcoming appointments, including providing event location details and landmarks.

Follow up with patients for rescheduling due to cancellations or no-shows.

Document outcomes for 100% of patient calls in the WellMed system.

Monitor call frequency, ensuring patients are not contacted more than six times within 60 days.

Utilize telephonic software for all outreach calls and manage inbound patient calls as needed.

Follow approved PCC call scripts for consistent patient engagement during outreach.

Support assigned PSU providers by scheduling patient follow-up appointments as requested.

Respond to patient messages, concerns, and information requests, ensuring that all inquiries are directed to the appropriate personnel within 48 hours.

Meet daily productivity targets to support PSU goals.

Establish effective and professional relationships with patients, colleagues, and the public.

Perform additional duties as assigned to support department objectives.

Requirements

High school diploma or GED required.

Strong communication and customer service skills for both in-person and phone interactions; basic Microsoft Office proficiency.

1-2 years of related work experience, including data entry.

Familiarity with ICD-10 and CPT coding.

Experience with electronic medical records (EMR) systems.

Basic knowledge of medical terminology.
