Description Location: 3801 Constitution Dr, El Paso, TX 79912 and/or 11820 Miriam Dr, El Paso, TX 79936Hours: Monday - Friday, 8:00 AM - 5:00 PM (Training during standard hours)Job SummaryWe are seeking an Associate Patient Care Coordinator (PCC) to join our team. This role is pivotal in ensuring patient outreach and scheduling for annual quality measure screenings, supporting the goals of our Practice Support Unit (PSU). The PCC will manage telephonic patient outreach, patient registration, and scheduling for diagnostic and follow-up appointments.Key Responsibilities

Patient Outreach and Scheduling: Perform outbound calls to schedule patients for PSU health events, ensuring they receive annual screenings for quality measures.

Data Collection and Verification: Collect and verify patient demographic and insurance information, including insurance eligibility, co-insurance details, and contact preferences.

Appointment Notifications and Reminders: Provide timely reminders to patients about upcoming appointments, including PSU location details and identifiable landmarks to assist with directions.

Follow-up and Rescheduling: Manage follow-up calls to reschedule any appointments due to cancellations, rescheduling, or patient no-shows.

Call Disposition and Documentation: Record outcomes for 100% of patient calls within the WellMed system, monitoring frequency to prevent more than six outreach attempts within a 60-day period.

Inbound and Outbound Call Management: Use telephonic software to manage outreach calls and respond to inbound calls from patients who may need assistance or have follow-up inquiries.

Compliance with Call Scripts: Adhere to approved PCC scripts for patient outreach calls, ensuring consistent and compliant communication.

Provider Support for Follow-ups: Support individual PSU providers with scheduling and follow-up requests to ensure patients receive timely care.

Message and Concern Resolution: Address patient messages, concerns, and requests, directing them to the appropriate staff member and ensuring a 48-hour response turnaround for 100% of inquiries.

Productivity and Professional Relations: Meet established daily productivity goals and maintain effective, professional relationships with patients, colleagues, and vendors.

Additional Duties: Perform other related tasks as assigned to contribute to team goals.

Requirements

Minimum Qualifications

Education: High school diploma or GED required.

Skills: Strong communication and customer service skills, both over the phone and in person. Familiarity with general office procedures.

Software Skills: Basic knowledge of Microsoft Excel and experience with Electronic Medical Records (EMR).

Preferred Qualifications

Experience: 1-2 years of related work experience, especially in data entry.

Technical Skills: Familiarity with ICD-10 and CPT coding, medical terminology, and prior EMR application experience.
