Job Summary - Care Coordinator LVNHillcrest Cancer Center - 150 Hillcrest Medical Blvd Waco 76712Schedule is Full time, Weekdays. M-F, 8a-5pAs a Care Coordinator LVN, you assist a Registered Nurse with patient care assessment and coordination using guidelines. You support clinicians and educators to achieve optimal clinical results by managing care efficiently. Your role helps reduce unnecessary service use and maintain high patient care levels.What You'll Do

You will follow a detailed plan to assess, implement, document, and update patient care plans. You will work closely with the health care team to provide evidence-based care.

You will coordinate patient care. Manage treatment goals and plans. Assess patients and families. Meet their health needs.

Your main duty is to help patients manage their health through education, visits, and phone calls, supporting their care plans.

You are expected to research, evaluate, and recommend resources to meet both medical and non-medical needs of patients and families.

You must communicate across programs to coordinate services and act as a liaison to healthcare facilities.

What Will Make You Successful In The Role

Understanding the basics of both care management and how to practically use resources is vital.

You must monitor, evaluate, and document patient progress to ensure care plan adherence.

Part of your role is helping patients make use of community resources. This means you must be facilitating by nature.

Building relationships is key - you'll work closely with both our internal medical staff and a range of external partners.

You will encounter high-pressure situations. Being able to adjust to different circumstances and respond to crises calmly is essential.

You will handle sensitive patient information and must maintain confidentiality according to policies.

You need basic computer skills like using software apps, data entry, managing medical records, scanning documents, and email.

Grad of an Accredited Program

Specialization in the field of nursing

(2) Two years of professional experience as an LVN

Licensed Vocational Nurse
