Care NavigatorJob DescriptionIn this role, you will be responsible for developing, assessing, and coordinating care management activities based on member needs to ensure quality, cost-effective healthcare outcomes. Your key duties include developing personalized care plans/service plans, educating members and their families/caregivers on services and benefit options, and advocating for improved healthcare access. You will also coordinate between the member and/or family/caregivers and the care provider team to ensure timely access to identified care or services. In addition, you may be required to conduct on-site visits to assess member’s needs and collaborate with providers or resources, and provide education to care managers and/or members and their families/caregivers on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits.Hard Skills
