Work location: Texas Health Harris 1301 Pennsylvania Ave Fort Worth, TX 76104

Work hours: Full Time, 40 hours, Days; M-F 07:30a-4pm

Responsible for coordinating the clerical and administrative processes for entity care transitions managementResponsible for managing deparmental mail box, phone lines and/or fax 30%Supports transition planning activities as needed under the direction of care transition managers :Calls vendors, physician offices, clinics, etcFaxes (electronic and traditional) documents as neededMakes copies or gathers other data as needed35%Collects and enters data as neededCompletes other tasks and projects as assigned 35%EducationH.S. Diploma or Equivalent ReqExperience3 Years relevant experience, preferably in a healthcare or service environment.
