Responsible for coordinating the clerical and administrative processes for entity care transitions management

Responsible for managing departmental mailbox, phone lines and/or fax

Supports transition planning activities as needed under the direction of care transition managers: Calls vendors, physician offices, clinics, etc.; Faxes (electronic and traditional) documents as needed

Schedule follow-up appointments, call Insurance companies for referral approvals, and sets up transportation for patients

Assist/support the Case Managers – will be assigned to 3 to 4 floors of patients – will go into the patient’s rooms for signatures on documents, as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs, walkers, wound vacs) to the patients prior to discharge

Makes copies or gathers other data as needed

Collects and enters data as needed

Completes other tasks and projects as assigned
