Outreaches are performed through an auto-dialer system
Documents all patient interactions in a concise manner that is compliant with documentation requirements established in Job Aids
Receives out-bound calls through an auto-dialer for program requirements including, but not limited to patient appointment scheduling, provider outreach, assess for special needs, Texas Health Steps and access to care for farmworker children
Schedules and coordinates patient transportation, and follow-up for physician appointments as needed
Provides clerical and / or administrative support to clinical staff and managers for special projects and reports
Provides excellent customer service by serving as a resource to all internal and external customers
Maintains knowledge of all health plan benefits, network, regulations, HHSC requirements
Informs members of available community resources available to help meet their needs, including but not limited to: TANF, SNAP, CSHCN, WIC
Complete appropriate assessments and care plans as needed to streamline patient access to care
Submits internal and external referrals
Assist all patients with barriers to care
Performs all other related duties as assigned
Meet and maintain metrics daily

High School Diploma / GED (or higher) OR 4+ years of medical customer service experience
2+ years of customer service experience
2+ years of experience in medical customer service
Intermediate level of computer proficiency including the use of Microsoft Outlook, Teams, Excel, Word, and the ability to navigate through multiple web applications
Reside in the state of Texas
Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications:
Experience working within the health care industry and with health insurance
Experience working in a hospital, physician's office or medical clinic setting
CNA or MA experience
Experience working in a call center
Frequent speaking, listening using a headset, sitting, use of hands/fingers across keyboard or mouse, long periods working at a computer
Health Care/Insurance environment (familiarity with medical terminology)
Ask appropriate questions and listen actively while documenting required information in computer systems
Bilingual English/Spanish
